I input an ancestral greater affixed Mace of King Leoric (as necromancer) and transmuted Upgrade to Mythic, and the result was The Gloom Ward shield. The Horadric cube description states that it will result in a random mythic from the same item slot. Weapons and shields cannot be equipped in the same slot so I feel like this is a coding mistake and I have now lost those pandemonium fragments.
