Having extreme difficulty picking up items or selecting actions with X button. on PS5

I’m having issues with barely being able to pick up items with the x button. it triggers the skill the vast majority of the time instead. If I run around and reposition and hammer the button, it eventually works. Needless to say, this is beyond frustrating. Tried more than one controller and it’s not hardware related. Any help?
