When I upgrade a weapon/item, I drag the item into the vendor’s upgrade tab box, confirm the new item in the lower preview box next to the list of upgrade material costs, then I hit the upgrade button. There is no way to accidentally dismantle anything using this method.
If you see the hammer icon while hovering over your inventory item, then you’re doing it wrong.

I accidentally sold my main crossbow last night while at the weapon vendor and thought I was swapping it with Windforce in my inventory. I did not realize what I had done until after searching for the missing crossbow, then making the mistake of logging off before checking the weapon vendor. Serves me right for playing sleepy lol.

A former co-worker loved saying “it was a user interface error” to the affected customer when that person caused the hardware break that landed them at the tech support bar. He said this with a completely straight face, and the guy didn’t know any better.

I think you’re upgrading “wrong” for lack of a better term.
Go to the Blacksmith.
Select the Upgrade tab.
Right click the item in question.
Click Upgrade.
It should not be possible to confuse Salvaging and Upgrading if you follow these steps.

This is what happened to me.
On the Xbox, when looking at your inventory, you press ‘X’ to mark for trash, and ‘A’ to swap item with what is in slot.
However, if you are at your stash, if you press ‘X’ it actually swaps weapons instead.
As such, when clearing out inventory at your stash, don’t highlight stuff for trash. I made this mistake and did not pay attention, as I had 2 items that looked similar, and when I went to the blacksmith, I detroyed the wrong item because it got switched.
Lesson learned, but I do not know why they would have a different interface for stash and inventory.

How!? There are no vendors near the stash.
