Mobs that perpetually runs back is on the top of my list. Another variation is teleporting elite that teleports offscreen and then resets.

Cancerous NM affixes that basically tells you to get a group if you wanna do it. Like… resource burns, backstabbers, all these ‘rofl you got one-shotted by environment while you were busy trying to burst mob packs’ affixes and there are things that disable your potions, deletes your resistances, reduces armors or negates your damage to near fullest. I mean, why? Isn’t a single troll ranged mob being able to one shot you off-screen enough? Leaves me no desire to do NM dungeons when I see majority of affixes and feel compelled to have groupies unless I roll a build that relies on cooldowns that are mindnumbingly boring for me.

Poison, Cold, Electrified Obelisks, Fire. Poison, Electrified Obelisks, Fire deal a bit too much damage. Poison/Obelisks doubles as CC too. There are times I get stuck in Obelisks and died because I was unable to move or hit anything. Poison pools are annoying because it leaves behind pools that lasts long and it deals like 5% of health per tick or something just delay Cold… cc galore on higher difficulties.

Backtracking. Goodness, I hate having to collect things just to progress the dungeon. Bonus point if you find needed items first. Since you can only carry 1 at a time, time to get back to the second one. Mindnumbingly boring.
