Azmodan Summoning, T4, Reward Chest fails to show up after 3x runs in the area.
Ran each of the Azmodan summonings solo, starting with Andariel, then Duriel & finally Belial. Repeated the loop for farming & I was unable to open the next chest (Andariel). Tried this 2 other ways, each starting with a different boss & running four times – the fourth chest did not open regardless of the boss. Did not leave the area between summonings & was dumping items in the area.
