Every now and then I notice that my wolf companion button get’s grayed out/disabled. Is this because it got killed or because it wandered off to smell some monster butts and is now too far away?
Maybe you have NumLocked the skill.
Yes, it is because it died. It won’t ever be too far, it will teleport to you if you get beyond its leash.
It doesn’t happen often, but if your pet has a health bar, it can take damage and by extention, die.
I’m not sure of the respawn time, but I have never seen it inactive for more than 5 seconds tops.

Pets toughness scales with your toughness. You can use enforcer in some build to make pets more tough.
