I have not played since D2 came out decades ago… Are there still plenty of people playing D3? Just looking for some people to play with casually? Anyway not sure what to purchase (expansion?) ??

There are still a fair number of players, but keep in mind that the game is nearly a decade old. Most of the players are old-timers. That means that there aren’t a whole lot of new players starting the game, so if you are playing in non-season, you will be on your own for the start of the game. You won’t find many others leveling up at the same time you are. Most public games will be on the highest difficulty level.
If you start playing seasons at the beginning, there will be a lot of players also starting off, so it won’t be too difficult to find a public game. Beware that most of them are still veteran players, trying to level to 70 as fast as possible, and expect you to keep up.
If you enjoy playing solo, then you can take your time.
You need to get the Reaper of Souls expansion to get anything out of the game. The base D3 is quite limited. The Necro character pack is completely optional.

There will be a new season starting April 2. At that point, alot of people will be restarting/gearing up again.
If you decide to buy, you pretty much will need to buy both D3 and Reaper of Souls as very few play D3 (with a level cap of 60) versus RoS with a level cap of 70. The necromancer class is a DLC for RoS. If you plan to take the plunge. I would also buy it with the necromancer too.
