I’ve noticed this before on other diablo 3 characters however have not tested each. Just after turning in from a Greater Rift (mine was empowered but I don’t think it matters) if you right away talk to Kadala, it appears as if the screen darkens a little and you can still navigate some on screen selections using the controller however you are effectively stuck at this point. Nothing but an escape to the PS4 home page and force closure of the app, and start again, can resolve it. At least as far as I have been able to determine.
