Meteors are not falling when two required items are worn.
Make sure you’re actually hitting/damaging an enemy with a skill or the Tal Rasha 2 piece bonus meteors won’t fall. Also, make sure that you alternate between different elemental attacks, as once a meteor of a specific element falls via the set, another meteor won’t fall, unless it’s a meteor of a different element from the previous meteor.
So if you’re using Archon, you won’t be able to call down different elemental meteor since all of the archon’s abilities will be of the same element, as a result you won’t be able to use the Tal-Rasha 2 piece bonus until the archon form expires. The only exceptions is if you also have a conjuration skill active that’s of a different element than the archon form you’re using, for example storm armor and familiar.

Of course, this is all assuming that you have the meteor skill on your skillbar. If not, then Tal Rasha’s 2 piece bonus effect won’t call down meteors at all.

Occam’s razor suggests you don’t have Meteor on your skill bar.
