I cannot get the portal to the Unhallowed Essence dungeon, to get the achievement: Set Me Free, to open. I have the 6 piece Demon Hunter armor set equipped, I’m on difficulty: Torment VII, I’ve been to the place between Caldeum and the Howling Plateau near the broken cart, and no portal opens up. What is the issue?
Not a bug.
The reason that the Unhallowed Essence dungeon portal isn’t opening is because your seasonal Demon Hunter - Heartseeker - is wearing six pieces of the Shadow Mantle set, not six pieces of the Unhallowed Essence set.
Each set (except GoD) has a set dungeon associated with it and the entrance portal will only appear if your DH has the six-piece bonus of that set in effect (this can either be 6 pieces worn, or 5 pieces worn with a Ring of Royal Grandeur). So, if you want to do the Unhallowed Essence dungeon, you need to be wearing the Unhallowed Essence set armour.
