Can I use reforge to change a set to a primal?
Yes. But, it is very expensive at 50 Forgotten Souls per roll. (You might want to collect 20,000+ Souls before you start).
Even upgrading Level 70 Rare items could be expensive at 25 Death’s Breaths per roll. Especially since you would have to roll a Primal version of the Set item that you want.
Your best bet would be to just farm for it on the highest Difficulty you can clear Areas fast without taking fatal Damage. Your chances are just as good, it would take you about the same amount of time and it’s the least expensive way. (You would be acquiring currency rather than spending it).

I play on PS4 non-seasonal HC so I guess you can imagine crafting isn’t a problem.
Can any legendary/set be reforged into a primal? For example endless walk set.
Yes, I believe so. (I’m not familiar with console play though). Should be the same as PC.
Note: As long as you’ve unlocked Primal drops.
Yes, exception items crafted by blacksmith and by Jeweler.
Craftable items can’t be reforged.
In fact, you can get primal everywhere in the game except one, the “Convert Set Item” recipe, which guarantees no ancient.
