I thought that the hardcore journey and normal mode journey were separate in past seasons. When did they combine?
The journey has always been a combination. The only difference between the two are the conquests, and those remain separated.

plus you can do the sc version of the h-gifts and claim on hard core
Not exactly. If you finish a chapter in SC, it’s forever locked in SC, and unclaimable in HC. What people tend to do though is leave something like transmog undone in SC, and complete the rest of the chapter requirements, so that Chapters 2-4 unlock in HC when they finally complete Chapter 2.

I always did everything in sc and claimed in HC, i never had a problem yet
I thought thats how it was, unless something changed…
hence the phrase (only claimable once per season) since you can claim it only one time on either HC or SC
