As I understand I might’ve missed a portal to Vision of Enmity during a dungeon with the cows event. I’m almost level 70 and not a single other portal opened. Am I missing something?
You’ll only get one at a time, but if you exit the game and start a new one it resets. If you did all 70 levels inside the same game, yeah, that’s why you didn’t get a second.
They spawn pretty often in the open world even when below 70. Try and go to the temple of the first born in act2. Be prepared to get hammered!
And put a ruby in your weapon socket…

What do you mean by that? Are you talking about a (rare) “cow rift” (it’s a nephalem rift that happens to be all-cows)? If so, then you surely are missing something - Vision portals do not spawn in rifts. If you are, however, talking about the Cow Level (opened in the wild by a goblin, or by using a Bovine Bardiche in the cube), and not getting a Vision portal inside it, then yeah, unlucky.
