Seems like this is a known issue since 2023 according to similar threads. But I’d like to share this because it was unexpected for me and I may have an idea about the issue. I have heard that WoL Monks causing Gelatinous Sires to disappear with a flick of their wrists but this is new.
Anyway, last night, according to EU time, I was playing D3 and entered a Vision. Only after a few floors I encountered three Rift Guardians and a trash mob, a wolf, to spawn the portal for next floor. I applied Exploding Palm manually for Mythic Rhythm to the trash mob instead of Rift Guardians which was a mistake on my part and now as you can see from the picture; there’s no exit portal, nor any other mob spawned for one.
My monk setup was an experimental build and I did not think that it’d be somewhat overpowered for a Vision of Enmity like that. The setup I use in the picture is rather unconventional Uliana setup. At that floor scale I knew I couldn’t kill three RGs without Rhythm bonus but didn’t expect to vaporize anything with less than 800 paragon.
Looking at the picture again, I have noticed that I have Nephalem Glory at the third stack which could contribute to the problem. According to my own knowledge, NG stacks can have weirdly overpowered interactions with Area Damage and Anguish shard applying explosions from slain monsters, the damage can be devastatingly high. I doubt if I post this in the Bug Report forums, anything will be done about it. Think this thread as a heads up for not doing any overkill moves against trash mobs in Visions, especially when in possession of Nephalem Glory stacks.
Anyone else encountered this issue in this Season?

The problem isn’t related to damage… I’ve had it happen with fairly low DPS builds. Rather it’s related to the monster that spawns the portal on death.
There are two ways this can fail. The most annoying one are monsters (like the flyers that swoop down on you when you walk over their trigger point) and the trigger point is outside the area you can visit. This happens about once a season to me in a couple of maps like the tight spiral map or the corrupted heaven map (which has an area near the bottom you can’t walk).
They other way you can have this happen is too high a movement speed… basically your speed is so great you effectively teleport over the one pixel hit box that triggers their arrival.
I got it to trigger once by going to the Paragon screen and resetting the movement bonus to 0. Which is super annoying.

As I have written in the original post, floor had placed a wolf as a trash mob but it seem to be bugged out due to some interferences between Nephalem Glory and Soul Shard theme. You might not see its corpse because it must be flung over out of the area or faded from view because I took a picture a bit late.
What I should’ve said is “I’ve never seen it happen from damage”. For me it’s always been the monster not appearing. It’s happened maybe a dozen times since visions became a thing. I just expect it to happen now.

Anyway as for OP, maybe it was a burrowing mob?
Sometimes they’ll just sit under the floor and absolutely refuse to come out unless you sit there for a while
Flying Demons also have the problem of refusing to land and appear in visions a lot, but i’ve never seen them be a trigger for the next level, burrowing leapers though definitely have
This season I had this happen a few times, killed everything, no portal or goblin. But I was told in-game that this has been going on for a long time. I guess I’ve been lucky until this season lol

If you run around, slain everything and game seem to “forget” the monster that could spawn the next floor portal, just roam around map one more time to encounter the last one. Game always wants to pick different type of monster to spawn the next floor portal and end it with a goblin spawn. But sometimes, in large maps, you supposed find a single monster which wasn’t any different than any other monster due to the monster types that have been predetermined on the generated floor map. This is kind of similar to Echoing Nightmares with dynamic spawns.
Let’s say game wants to pick a random Cultist to spawn the next floor portal, but the floor of Vision already have that monster type in spawn batches. With that case, you might witness that none of the Cultists you kill spawn the next floor portal, but upon roaming, you’ll find one that you “seemingly” missed and it’ll spawn the next floor portal when slain.
Appears to me, the game supposed to spawn a different non-Elite monster apart from the types of spawn batches, but in practice they mend into another group so it doesn’t trigger. When that happens, I think game just spawns something out of your view somewhere to trigger the next floor portal.
It also could be a Demonic Hellflyer or Burrower like described by above posters and you could be really unlucky.
It was a simple wolf enemy but things went awry by the way I described. You are free to believe what you want.
