After I upgraded to the latest Diablo 3 season, I noticed I now have a New Fuzzy White rectangle game icon. Looks like it might have a very faint blue pattern in the middle of the square with rounded corners. I can live with it, just wondering… (M4 iMac latest OS 24 gig memory)

I had the same issue. This is what I did to fix it on an M1 iMac running Sequoia 15.3.1 (the steps should work for all macOS versions):
1.) Open Applications folder in the Finder.
2.) Open Diablo III folder from Applications folder.
3.) Select the “Diablo III Launcher.app” (it still has the regular icon).
4.) Get info on “Diablo III Launcher.app” by going to the File menu in the Finder and click Get Info and then click once on the icon at the top left of its Get info window.
5.) Go to the Edit menu in Finder and click on Copy.
6.) Directly underneath “Diablo III Launcher.app” is the “Diablo III.app”. Click on “Diablo III.app” once and Get Info on it from the Finder.
7.) Click the white icon at the top left of the Get Info window for “Diablo III.app” and then go to Edit menu in the Finder and click Paste. Then close the Get Info window for both applications.
8.) When you launch Diablo III from Battle.net app it may ask you to update Diablo III. Click “Update” and WAIT for it to completely update before clicking on the “Play” button. Waiting to hit the “Play” button helps avoid data corruption.
