I see that this issue has been discussed in 2019 and 2020. And unfortuantely, it’s still around.
I have played a few chars before (sader, DH, a bit of necro and WD) the last 2 seasons and hadn’t encounted this before (or I didn’t notice). With my Tal sorc, I’ve now run into it at least thrice, possible more often without noticing. Does anyone know what causes it and how to avoid it?
Would the issue be solved if I switch to another set as soon as I have the 5 items together?
As far as I get it; every entity has to be removed from the game field when they are slain, but game refuses to do it outright when they die with something that affect their positioning. So game director, or that function what-not, awaits the position of monster or entity to become precise before removing them. As entity or monster stuck in “limbo” and its position never become precise because it supposed to die, it relentlessly chases you.
Be sure you don’t have any knockback, push effects or position manipulating item, as it can clash with any pull effect you have (for example; Black Hole) to cause this. Sometimes Nephalem Glory pops at its end of duration as you try to pull stuff as well, but no idea if it contributes to it or not.
Per my experience, this usually happens with small frame Burrowers, Scavengers that dig through the ground or Winged Demons that vanish to the sky to become invulnerable in an instant at their first confrontation due positioning. While fighting them any push or pull effect with Area Damage may kill them as they try to avoid, and this might result with this issue to pester you.
Best course of action is just avoiding the invulnerable monster and changing area or causing it to get charmed (hint: switch Enchantress’ abilities or equip Overwhelming Desire amulet while farming) as you ditch it.
Terror Demons? Just skip and move on.
If I remember correctly it’s a problem if they can’t be killed fast enough?
Yeah, they’re a candidate for this issue as well. I have encountered Burrowers and Scavengers as they stay idle beneath the terrain, but Terror Demons are just too much to deal with.
First time was act 1, doing bounties. Had to kill skeletons which spawned form some jar. One of them refused to keel over.
2nd and 3rd were blue bosses in GRs which prevented me from getting their orbs. Don’t recall which monster rype it was.

… sometimes it’s just a visual bug. Meaning the elite keeps standing motionless after being killed.
but no progress orbs dropped. Both times.
Just hope that you don’t get the bug where the diabolical chest in a vision spawns at an unreachable place!
Not YET. But today and yesterday make me feel like that’s what wil happen with the next vision I do.
I’ve just found The Smoldering Core (after the three bug encounters, so that wasn’t the cause). That makes me wonder if I should better either not use it or drop Black Hole from my skill bar …
That means something triggered it. Maybe it’s just because those Jar of Souls event skeletons emerge from the ground and you killed them as they spawn, not allowing them to reset their state.
That’s not a vector pull or push effect, it’s just taunt where monsters run to the target on their own accord. I don’t think that’s the cause. Because if that’s the case then this bug would haunt Natalya Spike Trap DH, more than Meteor Wizards.
You know you should not do that; it’s a staple skill for Tal Rasha Meteor build. I say, just change the Teleport skill rune to Safe Passage instead of Calamity as its emitting Wave of Force can be problematic in my opinion and note your findings. Change it back to Calamity skill rune in Greater Rifts because it will cost you damage output (it fills the role of Arcane damage in Elemental Exposure) and high Greater Rift monsters are not fragile as T16 ones to die to a breeze.
On another note, maybe just toss Smoldering in the Cube because the current one you have very low minimum damage anyway. Look for a damage modifier close to +1870-2325 range, yours currently have +1664 as its min dmg modifier.
These bugs, or whatever they are, are so rare that I’ll advice that you don’t change anything. Of course you can use different skills to keep up the 4 elemental Tal Rasha stacks depending on what variation of Tal Rasha you use and what activity you do.
I thought it was a bug too but then it occurred to me… isn’t it just illusionist?
Like outside a gate or halfway into a wall. I have seen that a few times. I try not to corner that goblin because of that.
