Had something similar. Just had him disappear on me after I hit him once as a DH playing shadow’s mantle. Had to keep hitting below a floating health bar to make him reappear in his next phase. Weird.
Same thing happened to me. I was playing a Shadow DH and completing the Season Journey Chapter IV Objective, Only You. During the battle with Malthael, not only would he disappear occasionally, but so did my Character, Follower and Companion. Even after I killed Malthael, every few seconds the video would glitch. Before returning to town I decided to capture a screenshot of it.
After a few seconds it would clear up. And if I waited long enough it would do it again.
