Item disappeared when using armory

Were you disconnected from the game shortly after finding them (the first time)? I ran into a strange problem today

I was in a vault, I had just killed the Greed and opened the chest and I was disconnected.
When I relaunch a game, I had in my inventory the items that I had deleted before the vault, and the puzzle ring in my stashes… And I’m sure I only had one.
