I think they just use tiers to distinguish relative strengths of builds based on GR progression. However, this isn’t always the full story as some builds start to shine at very high paragon (e.g. Natalya’s RF) since you need a lot of toughness to make it viable and then beats other builds.
Also, Icy-Veins is prone to errors in the details in many builds. They often omit alternatives (e.g. Gizzard gem for new UE Multishot) and mention old options (e.g. Witching Hour for the new UE Multishot) which cannot be used with current setups (Witching Hour cannot be used if one wants to use Captain Crimson).
so where do I find Good Setups around the internet?
Well Icy-Veins is a good starting resource and their information is 99% correct usually. Just some of the most subtle and advanced concepts might be a little outdated or misinformed. These little errors won’t affect casual players but a more experienced player will maybe want to optimize their build and these little things can make a difference.
Another example I read was in their LoN/LoD Rapid Fire guide in which they suggest Rapid Fire +% on their equipment. Unfortunately, since the Wojahnni Assault bonus provides a huge 3000% additive bonus to the “damage increased by skills” (a.k.a. DIBS) category, adding in another 15~45% will be negligible and it would be better to have some other affix in those spots (e.g. vitality).
Top tier 130/140+
High tier 120+
Mid tier 115+
Low tier 110 or below
Note some build/classes are stronger than others. These are estimates and factor in strongest solo builds/end game paragon.
It also very much depends on how dedicated people are. Even with a “top tier” build, you won’t clear 120, let alone 130/140 unless you know what you’re doing and spend a lot of time playing to get GG gear and paragon levels.
As for the low tier, clearing 100 with some of those would be a real challenge.

Would it be a good idea to use D3 planner to tweak Icy-Veins builds to optimize it for damage and survivability?
Once you decide on a class/build, go to the class specific forum for more detailed information.
D3 Planner is a great resource for fine tuning a build.
