On two occasions tonight our party had completed the greater rift, leveled up our gems and could not talk to orek for completion and had to remake game.
The only time I have ever heard of this happening was because one or more players still had at least one more upgrade on their LGems. Make sure everybody double-checks that they do not have an upgrade still pending with Urshi.
Tip: When I play in groups I always use Town Portal to return to Orek, (instead of just using the Map Waypoint). That way, if Orek is inaccessible, I can port back to Urshi and let the other players port to me to do a double-check.
