Something has glitched and I no longer have flawless victory. I completed Avarice, Boss Mode & Thrill, it shows in the leaderboard that I completed them, but Flawless Victory remains unachieved. I’ve done the work and should be awarded with the season rewards. This is the first season I’ve ever completed and it’s so disappointing to not get the reward. What’s the point of all grinding for the achievements if the rewards aren’t actually awarded?
The conquest is different from the achievement. For the achievement, you are allowed to wear green items as long as you do not have a set bonus.
For the conquest, you can not wear ANY green items at all, even if they provide no set bonus.
This is not a bug, and is part of the description of the conquest. I am going to assume this is the problem, since it is usually where the confusion lies.
No it is a bug. I didn’t wear any set item period. I was momentarily awarded flawless victory, because I completed the 3 conquests, and then the next time I logged on it’s gone and says I still need to complete Flawless Victory, again. But it shows on the leaderboard that I completed those 3 conquests.

The only suggestion I have is to attempt it again, or to try to complete another Conquest. Sorry it is not working for you.
