whenever you kill an elite pack with BotP equip, you would get a small icon showing how much time you have left from the gem.
i have noticed that once in a while when i ever kill an elite pack, BotP will not activate nor refresh. it doesn’t happen often but i noticed it more frequently on Greater Rifts. i am wondering if this is just a visual icon error or something more.

I haven’t noticed any issues with this gem personally.
My guess is you are killing a yellow elite, and finding it doesn’t proc. This is because you need to kill the entire pack including his white trash buddies (minions). Not just the big bad elite.
If this isn’t the case, then perhaps trying to replicate the situation you found this in might shed some light.
you are prob right. i havent used this gem for a very long time so i may have been too quick to jump the gun
