I played with a friend, and they got their ring of royal grandeur. ALL I got was plans. I have since gotten no more plans, nor act items, nor blood shards.
A RoRG isn’t guaranteed even at Torment XVI with double caches. Nor is any bounty act legendary.
After a while you will acquire all the plans and they’ll stop dropping. Do you have space for more shards or have you hit your cap?
This link is older but it still checks out. It’s not until you’re doing T6 or higher that you’re guaranteed a legendary within a cache and even then that’s out of the pool of legendaries for that act.

Fun fact, I levelled a toon for funsies yesterday afternoon (yes, I wasn’t going to play this season but hey, I was at a loose end literally waiting for paint to dry) and got a RoRG from a T1 Act 4 cache about level 40-something or so. Don’t think I’ve ever had one so soon. Definitely don’t remember ever getting one pre-70.
