Happened on seasonal character, char name was Inkheary763; kinda heart breaking, but the achievement for seasonal rank completion is bugged for person and cannot complete?
You have to have the 3 gems in your stash.
If you transferred one to another toon, the game don’t see you have three gems lvl35(+).

I had the same thing happen (and I only had one toon at the time). I just kept playing and at a later point, I think when I had leveled 3 gems to 55, I got all completions.
They have to be accessible to the character at the time you level them. So in your inventory (socketed counts) or in your stash. As a bonus the Whispers of Atonement from the Echoing Nightmare count and it’s easy to get them to rank 125. I usually put three in my stash and the next time I level anything at Urshi all the remaining season achievement fire off in succession.
