I’ve completed the entire journey, but the Azmodan, kill in 30 sec section won’t check off, although I’ve done it in secs through t13 - t16. Maybe it’ll check off after I log out for a while. Guess I"ll check tomorrow.
Same issue. Reloaded, and I tried first dying to Azmodan, then respawning at last checkpoint and trying again. That ended up correctly checking off the quest.

This gets reported each season on the regular bug forums. It is caused by killing him too quickly.
Let him fully spawn in and make sure he is done talking, then let him make the first move before attacking him. This usually works, but you can also try reducing your damage so it takes more than one hit to kill him.
Most people do not encounter this because they are not strong enough to one shot him when they kill him this early in the game.

I can confirm that. The same thing happened to me. I reloaded the game, waited until the scene fully rendered, stepped back from him about 15 yards, then let him have it.
In for the Kill completed.
