Why not? Apparently in today’s society you can claim to be anything. But then claiming you are anything doesn’t always mean you truly be what you claim you are.
Women could be King. In fact there are an old series of fantasy books about Xanth, A Florida shaped mystical land where it would only be ruled by Kings. So… If a female ended up as a ruler, they simply became King. The rule was never broken as it didn’t specify gender.

The Cow King always had an udder. That is why D2R is made the same way. It has been remarked on over the years many many times. So, either 20+ years ago the Devs were ahead of the times, or it was just a funny little oddity in Diablo 2.
The Cow King has a male voice (and title), and is overall regarded as a male. However, it has an udder and has been stated to produce milk.[1]. To add even more confusion about the Cow King’s gender, two different creatures named The Cow Princess and The Cow Queen can be encountered in Diablo III, and neither of them is the Cow King (as his ghost can still be seen in the same level). The gender of these two creatures is unknown, as they never speak, and no other in-game characters speak of them, neither as ‘him’ nor as ‘her’.
You would have to ask the original Devs why the Cow King has an udder.

When the King dies for a credited kill to a character, he drops a random array of magical/set/unique items (usually five total) as well as a spray of stamina potions, in an homage to the milk he (she?) would produce were it not a mutated, sexually-ambiguous Hell Bovine.
