If you want a different look for your character, change the gear, thats always been the way.
I would welcome a basic character selection and/or customisation screen: skin colour, hair colour, facial hair etc
I would expect that in Diablo 4.
This is invalidated by the fact that Unique items look different from their base
Therefor lorewise, heroes cared about the look of their items to a point they altered them

Not all games have to be exactly the same in 2022.
Don’t know many games that have Anya and “item transmutation” built in the lore of the game
It’s not like it’s adding cosmetics to the game, it’s just using the already existing models
Big difference and stretch from adding some custom words to an item, vs changing the appearance at will.
Vanity right down to your game character… ya’ll need to be that special?
