Any mf gear on?
I’m sitting at 96%mf with gear I found organically and good stuff just rains down from heaven all the time.
Haven’t even made it into hell yet.
I did struggle a lot getting out of normal but to be honest the best bet is countess runs and some cows until you can you can make a rune word.
Other then that be on the lookout for blue items, also refreshing merchants. Like Drognan has pretty awesome Wands for example you can waste gold on by re-rolling the map. The best spawns seems to be from when Lut Gholein gateway is on the right and not the left. Ormus is the same with staves.
If you somehow end up bad too long but bulk on gold, last resort is truly gambling, boots / gloves are easy to get with Magic Find. This can increase your chance at obtaining better items.
But nightmare is not that complicated to be honest, it’s mostly demanding to to just take your time a bit more. (Hardcore players known what I’m talking about.) and investing more time into getting higher level so your skills actually with their synergies mostly do 2/3 of the job for you.

It’s not about “good items” dropping. It’s about making the most with what you can get to drop. Seriously, there are great low-level runewords to make. Stealth, Lore, Leaf, Rhyme, Spirit, Insight…
Farm countess in normal to make Stealth. You can also shop for bases in the first 2 acts of normal. You’re not going to get the best gear out of normal, but you can find some decent gear to take you into NM and beyond.
