I found it is the easiest to replicate when you just stand there with 1 slayer in the chamber and Nihlathak left. Kill any ice spawn that Nihlathak summons and let him keep “frenzying” the single Slayer (which visually turns into a skeleton). To make him shift back into the Slayer form, leave the area and come back
