I would recommend to try the following:
Only changes to the client I would recommend to keep:
- graphicsapi Direct3D9
- Keeping settings lowered to medium.
Out side of the game:

Reset the Nvidia Settings to default.

Keep background applications disabled following the Windows 10 steps Here and restart the PC.

Then make sure the Operating System and NVidia settings are set to high performance:

NVIDIA Steps: Right click on your Desktop → NVIDIA Control Panel → Manage 3D Settings → Under the Global Settings tab → Select: Power Management mode → Prefer Maximum Performance. Apply → Exit the window.
If the issue continues there are a few outstanding bugs related to performance but I don’t see any known for your CPU or GPU. Just to make sure we covered as much as we could double-check the system isn’t overheating.
